CHHA's are a valuable service for seniors or those who need some extra help at home. With certain insurances -like Medicare- CHHA is not covered as an individual service. But can be provided in addition with our other great services.

Our goal is to equip our clients and their families with the tools to cope with the medical challenges that are current or may occur in the future.
Social Worker services include, but are not limited to:
Identifying the social, emotional, and financial concerns of the patient/family
Assist with community resources and personal counseling
Arrange meal services and transportation to medical appointment
Connecting the patient/family to financial resources
Connecting the patient/family to relevant information
Helping develope long term plans for living and care

How it Works

REFERRAL
In order to begin, we need to receive a "Home Health order" from your physician. The doctor can include any specific instructions needed for care.

Patients can reach out to us to help answer any questions about getting a referral.

ELIGIBILITY
We can check your eligibility to find out exactly what is covered by your insurance. Our clients will receive a full explanation of benefits within a day to make sure we can begin care asap.
Patients with Medicare may be eligible for full coverage of services. Ask us how.

SCHEDULE
Once we receive your Home Health Request, our office will reach out to schedule your initial visit. We will confirm a time and place that fits with your schedule, and answer any questions.
The frequency can be scheduled for weekly, several times a week, or daily visits.
